How to Request a Site Number

The following is the information needed to request a site number from the Archaeological Records Office at the Arizona State Museum. Information NOT included in your request will delay the results. Please have the necessary information before you request a site number.

Please Supply

Fill in the Site Number Request FormOpens in a new window (MS Word - Help) with the following information:

  1. Quadrangle (ex. AZ A) - see ASM Site Numbering and Quad System and Quad Names and Numbers (PDF*)
  2. Rectangle (ex. 1–16)
  3. Quarter section of rectangle (ex. NW, NE, SW, SE)
  4. Project name or number
  5. Land ownership (of site)
  6. Number of site numbers need for each of the above quadrangles and rectangles, or per different quarter sections of one quadrangle and rectangle.
  7. Fax or email the information as shown on the form.

NOTICE:

If you request ASM site numbers we expect that the complete project will also be submitted. The standard procedure for an ASM project submission is that the site cards, project registration form, three copies of the report, and the project registration fee are submitted at the same time.

Completed Arizona State Museum Site Cards (or alternate forms) for all sites located or examined by the survey or excavation project must be submitted. Site cards are required to be updated for sites that have been previously recorded.
